    Balancing Between ‘De Facto’ and ‘De Jure’ in Standard-setting Strategy by a Latecomer Country: The Case of ICT Industry in China

    The main objective of this study is to gain insights into the complexities of the standard-setting process involving a latecomer country. Standards are identified into two categories: market (de facto) standards arising from market interactions and formal standards (de jure) arising from mandatory specifications by public authorities. The study focused on two research questions: (i) What are the different factors that influence the outcome (success or failure) of a proposed standard by a latecomer country? (ii) How does a latecomer country navigate between de facto standard and de jure standard? It analysed two cases (WAPI - a failure and TD-SCDMA - a success) to examine the extent of government support for a standard setting that needs to balance between the de facto standard and de jure standard. The study found that standard competition is a complicated interplay between technology, markets, politics and institutions. The experience of two cases suggest that a proposed standard should judiciously combine partly a ‘de jure standard’ which aims to protect national interests and achieve a national goal, and partly the ‘de facto standard’ to meet the technology trend, market demand, as well as the end users’ welfare.     DUET: A Generic Framework for Finding Special Quadratic Elements in Data Streams

    Finding special items, like heavy hitters, top-k, and persistent items, has always been a hot issue in data stream processing for web analysis. While data streams nowadays are usually high-dimensional, most prior works focus on special items according to a certain primary dimension and yield little insight into the correlations between dimensions. Therefore, we propose to find special quadratic elements to reveal close correlations. Based on the items mentioned above, we extend our problem to three applications related to heavy hitters, top-k, and persistent items, and design a generic framework DUET to process them. Besides, we analyze the error bound of our algorithm and conduct extensive experiments on four data sets. Our experimental results show that DUET can achieve 3.5 times higher throughput and three orders of magnitude lower average relative error compared with cutting-edge algorithms

    Anti-sulfatide antibody-related Guillain–Barré syndrome presenting with overlapping syndromes or severe pyramidal tract damage: a case report and literature review

    IntroductionAnti-sulfatide antibodies are key biomarkers for the diagnosis of Guillain–Barré syndrome (GBS). However, case reports on anti-sulfatide antibody-related GBS are rare, particularly for atypical cases.Case description, case 1A 63 years-old man presented with limb numbness and diplopia persisting for 2 weeks, with marked deterioration over the previous 4 days. His medical history included cerebral infarction, diabetes, and coronary atherosclerotic cardiomyopathy. Physical examination revealed limited movement in his left eye and diminished sensation in his extremities. Initial treatments included antiplatelet agents, cholesterol-lowering drugs, hypoglycemic agents, and medications to improve cerebral circulation. Despite this, his condition worsened, resulting in bilateral facial paralysis, delirium, ataxia, and decreased lower limb muscle strength. Treatment with intravenous high-dose immunoglobulin and dexamethasone resulted in gradual improvement. A 1 month follow-up revealed significant neurological sequelae.Case description, case 2A 53 years-old woman was admitted for adenomyosis and subsequently experienced sudden limb weakness, numbness, and pain that progressively worsened, presenting with diminished sensation and muscle strength in all limbs. High-dose intravenous immunoglobulin, vitamin B1, and mecobalamin were administered. At the 1 month follow-up, the patient still experienced limb numbness and difficulty walking. In both patients, albuminocytologic dissociation was found on cerebrospinal fluid (CSF) analysis, positive anti-sulfatide antibodies were detected in the CSF, and electromyography indicated peripheral nerve damage.ConclusionAnti-sulfatide antibody-related GBS can present with Miller–Fisher syndrome, brainstem encephalitis, or a combination of the two, along with severe pyramidal tract damage and residual neurological sequelae, thereby expanding the clinical profile of this GBS subtype. Anti-sulfatide antibodies are a crucial diagnostic biomarker. Further exploration of the pathophysiological mechanisms is necessary for precise treatment and improved prognosis

    Cumulative sum learning curve analysis of tubularized incised plate repair for hypospadias: a study of a single surgeon with a single surgical procedure

    PurposeTo ascertain the quantity of instances by which a single surgeon achieves competency and proficiency in using tubularized incised plate (TIP) technique for the repair of distal and mid-shaft hypospadias using the cumulative sum (CUSUM) analysis.MethodsWe retrospectively evaluated patients with distal and mid-shaft hypospadias who were treated by a single surgeon between 2015 and 2021, using a single primary TIP technique with a de-epithelialized Byars flap. Data including type of hypospadias, age at surgery, curvature, operation time (OT), length of the reconstructed urethra, and postoperative outcomes were collected and assessed. CUSUM was used to assess the trends in OT and complication rate (CR) in order to generate the learning curve. The evolution of OT and CR can be divided into three phases: learning, competence, and proficiency.ResultsCUSUM identified three phases in the learning curves of all TIP repairs. The median OT decreased from 135 min [interquartile range (IQR) = 125–155] to 92 min (IQR = 80–100) (P < 0.001), CR decreased from 28 (28%) to 8 (5.3%) (P < 0.001), and reoperations decreased from 15 (15.2%) to 4 (2.6%) (P < 0.001). According to the CUSUM learning curve, technical competency plateaued after the 99th case, and both OT and CR entered a significantly declining proficiency phase after the 231st case. Further, when the neourethral length exceeded the total average, total complications, urethrocutaneous fistula, and reoperations increased (P = 0.013, P = 0.006, and P = 0.028, respectively).ConclusionsOur study suggests that surgeons performing TIP repair may reach technical competency and achieve proficiency after operating on 99,231 cases, respectively.     Whole exome sequencing of insulinoma reveals recurrent T372R mutations in YY1

    Functional pancreatic neuroendocrine tumours (PNETs) are mainly represented by insulinoma, which secrete insulin independent of glucose and cause hypoglycaemia. The major genetic alterations in sporadic insulinomas are still unknown. Here we identify recurrent somatic T372R mutations in YY1 by whole exome sequencing of 10 sporadic insulinomas. Further screening in 103 additional insulinomas reveals this hotspot mutation in 30% (34/113) of all tumours. T372R mutation alters the expression of YY1 target genes in insulinomas. Clinically, the T372R mutation is associated with the later onset of tumours. Genotyping of YY1, a target of mTOR inhibitors, may contribute to medical treatment of insulinomas. Our findings highlight the importance of YY1 in pancreatic β-cells and may provide therapeutic targets for PNETs

    Genome Assembly and Annotation of a High-Polymalic Acid (PMLA) Producing Strain Aureobasidium melanogenum CGMCC18996 and Analysis of Its Key Proteins Related to PMLA Synthesis

    In this study, we applied PacBio Sequel II and Illumina NovaSeq 6000 sequencing platforms to sequence the genome of a high-polymalic acid (PMLA)-producing strain, Aureobasidium melanogenum CGMCC18996, and used different assemblers to obtain a high-quality genome assembly, which was then annotated using transcriptomic data. The results indicated a total of 6 202 genes were found in the A. melanogenum genome, mainly involved in carbohydrate transport and metabolism, amino acid transport and metabolism, post-translational modification, RNA processing and modification. Meanwhile, functional annotation revealed that most genes in the genome were related to peroxisome in the strain. Transmission electron microscopy (TEM) indicated the existence of a circular peroxisome-like (glyoxysome) structure in the cells, demonstrating the ability to malic acid through the glyoxylate cycle. Finally, we predicted the protein structures of two enzymes related to PMLA biosynthesis, phosphoenolpyruvate carboxykinase (PCKA) and malate synthase (MASY). It was found that the enzymes could have the ability to synthesize malic acid. This study could provide a reference for metabolism regulation in A. melanogenum for improved PMLA production, and the assembled genome has been uploaded to the database, which could provide the basis for the future development and utilization of A. melanogenum CGMCC18996

    A preliminary evaluation of targeted nanopore sequencing technology for the detection of Mycobacterium tuberculosis in bronchoalveolar lavage fluid specimens

    ObjectiveTo evaluate the efficacy of targeted nanopore sequencing technology for the detection of Mycobacterium tuberculosis(M.tb.) in bronchoalveolar lavage fluid(BALF) specimens.MethodsA prospective study was used to select 58 patients with suspected pulmonary tuberculosis(PTB) at Henan Chest Hospital from January to October 2022 for bronchoscopy, and BALF specimens were subjected to acid-fast bacilli(AFB) smear, Mycobacterium tuberculosis MGIT960 liquid culture, Gene Xpert MTB/RIF (Xpert MTB/RIF) and targeted nanopore sequencing (TNS) for the detection of M.tb., comparing the differences in the positive rates of the four methods for the detection of patients with different classifications.ResultsAmong 58 patients with suspected pulmonary tuberculosis, there were 48 patients with a final diagnosis of pulmonary tuberculosis. Using the clinical composite diagnosis as the reference gold standard, the sensitivity of AFB smear were 27.1% (95% CI: 15.3-41.8); for M.tb culture were 39.6% (95% CI: 25.8-54.7); for Xpert MTB/RIF were 56.2% (95% CI: 41.2-70.5); for TNS were 89.6% (95% CI: 77.3-96.5). Using BALF specimens Xpert MTB/RIF and/or M.tb. culture as the reference standard, TNS showed 100% (30/30) sensitivity. The sensitivity of NGS for pulmonary tuberculosis diagnosis was significantly higher than Xpert MTB/RIF, M.tb. culture, and AFB smear. Besides, P values of <0.05 were considered statistically significant.ConclusionUsing a clinical composite reference standard as a reference gold standard, TNS has the highest sensitivity and consistency with clinical diagnosis, and can rapidly and efficiently detect PTB in BALF specimens, which can aid to improve the early diagnosis of suspected tuberculosis patients
